Additionally, both parents are at or past the age of 40. Past age 40, not only can fertility be impacted but also pregnancy complications along with a higher incidence of birth defects. If OP's wife has any medical conditions, her pregnancy could be high risk at 40. So, if they are going to have another child, now is the time to do it.
While it may “feel” to OP like an ultimatum, OP also seems to have a cavalier attitude about another pregnancy as if it isn't a big deal. It is. And it was no secret either. OP knew from the beginning his wife wanted two children so he needs to either step up and keep the commitment or let his wife find someone who will.
In OP's defense, however, he may not be aware of the hazards and complications of a pregnancy past the age of 40. So he needs to go online to get a better understanding of his wife's concerns.
