She doesn't deserve to be back in your life after all the horrible things she has said about you. And its very telling that she only expects you to talk to her again because she thinks she can get something (your liver) from you.
As harsh as it might be, her health problems are literally her own fault. She's an alcoholic and nicotine addict with no motivation to change aside from her doctors telling her she needs to or else she could be very sick.
You can't make her be a better person and from the way you've described her I fully expect if she did need a transplant she would go right back to old behaviors after. Because she is refusing to address the root of the illness, and you can't make her, and her presence in your life will only make your life worse.

Sounds like to me like you need to divorce your husband and go live! with your sister and her husband. You are making excuses as to why you really don't want him to come with you and you also sound like you don't care. He is definitely not a priority to you. Just admit that you don't want your husband around you when you travel and neither does anyone else. Just divorce your husband and go move to where your sister lives and your problem will be solved , or let your husband take trips without you, and enjoy his life without you
