I think you have to accept that your boyfriend has some serious and persistent deficits. You can't fix him, and I'm not even sure he could fix him, even with therapy.
I'm curious what your physical disability is, and did you always have it or is it more recent?
She needs to take responsibility for her situation. Unreal that she is serious when telling you to gain weight! If you haven't, I suggest having a serious conversation about this or suggest therapy. She will likely resist holding herself accountable but something has to give.
