This is way above your pay grade. He needs professional help. The only way you can help is to support him as he goes to therapy and potentially starts taking medication. If he doesn't want professional help then there is nothing you can say or do that will make a difference. Intrusive thoughts to that level are a serious and complex mental health issue.
You cannot fix someone else. You cannot help someone who isn't actively helping themselves.
You could gently suggest therapy or that he talk to his GP about it. But it's up to him to make the next step and nagging him will just exhaust both of you and cause resentments.
Why is there a need to talk about it? Maybe you should just invite him to do things. If he says no often enough then dump him. You might want to consider dumping him now.
