You can't be his human support animal for this. You may also want to talk to a counsellor or therapist to get your anger out of your system.
Perhaps tell him you want absolutely no contact for 6 months. After that time IF he has followed the program and got his shit together, you may be interested in an update to see how he's doing.
Your trust in him is 100% gone. It will take a very long time to trust again unless he makes a super-human effort to deal with his issues. Even then, do not offer any guarantee that you'll reconcile. He needs to experience the consequences of his actions.
