Is he in therapy? You don’t mention it, but if he isn’t, he definitely should be. I don’t think this is something he can handle in his own.
I also don’t think you should feel responsible for helping him through this. This is some major mental health issue, and you’re not equipped.
I would put the sex on pause for now. My girl used to cut herself between her thighs near her crotch and I had no idea what to do in sexual situations. I think it’s totally normal you have these feelings.
I wouldn’t mention that to him though. Don’t make him feel bad about himself and don’t mention how the scars are a turn off. What you can do is tell him that this is obviously a serious medical episode and he needs to get help, that you’re worried about him.
I’ve been involved with a couple women with serious mental health issues and my best advice is if you don’t think you have the bandwidth to help them, it may be best to end it.
